Description

Whitepost is a post made of fiberglass composite and high-strength epoxy resin that acts as intraradicular reinforcement of the dental structure and promotes retention for the definitive restorative or core build-up material, in the case of indirect restorations. It is available in the DC and DC-E versions. With the same apical diameter corresponding to the DC version, Whitepost DC-E (Special) is different because it is thicker in the cervical region. It offers more strength in cases with a lower amount of remaining coronary portion or for teeth with wider canals.

Thus, the double tapered post fits to a double tapered preparation, producing the stability needed for the system and attaching to the root with less cement thickness, which favors its long-term longevity.

Indications

• The indication of an intraradicular fiberglass post is directly related to the need to restore an endodontically treated tooth to assist in retention of the restorative material and distribute stresses imposed on the tooth.
• In case of a tooth with significant coronal destruction from caries, trauma, or extensive restoration, it is recommended to place an intraradicular post to reinforce the coronal portion, thus minimizing the probability of fracture. It can be used with dual-curing and light-curing adhesive systems due to its high capacity to transmit light.

Step by Step

Step 1

Do the X-ray exam (periapical radiography) and check the quality of the endodontics. Select the post according to the diameter of the canal, overlapping the template over the X-ray of the tooth.

Step 2

Isolate the operative field. Prioritize the use of a rubber dam.

Step 3

Remove the gutta percha from the canal using a suitable drill or instrument, keeping 3 to 5 mm of gutta percha in the apical region, ensuring the sealing of the endodontic filling.

Step 4

Prepare the root canal with appropriate drills until it matches the shape of the post. Avoid unnecessary wear. For Whitepost DC posts, use the Whitepost DC drills 0.5, DC1, DC2, or DC3, according to the diameter of the post you are going to use.

Step 5

Place the selected post in position and X-ray to assess its fit and its positioning. The post must be perfectly adapted to the preparation performed, not presenting spaces between the end of the post and the gutta percha.

Step 6

Trim the coronal excess from the post, if necessary, and put it back in position to check the fitting.

Step 7

Apply the phosphoric acid gel (Condac 37 – FGM) for 15 seconds inside the root canal and remaining dental structure. Wash rigorously and remove the excess water with absorbent paper cones.

Step 8

Use the Cavibrush Long (FGM) for application of the dual-curing or light-curing adhesive system along the entire etched root canal.

Step 9

Apply Prosil Silane (FGM) on the entire surface of the post and wait for it to dry.

Step 10

With a lentulo drill, introduce the Allcem resin cement (FGM) inside the canal or use Allcem Core (FGM) with its applicator tip.

Step 11

Insert the post carefully in the canal and proceed with light-curing of the cement on the surface and through the post. Note that the post must be inserted down to the bottom of the prepared cavity and immobilized in cases of wider canals.

Notes

1 – If the canal is too wide, you can also cut the apex of the post to gain greater diameter in deeper regions of the canal.
2 – Do the cleaning of the Whitepost with 70% alcohol or 2% chlorhexidine. It is recommended to sterilize the post by autoclaving (121ºC and 15 psi) prior to cementation. The post must be very dry at the time of cementation.
3 – Use proper packaging during sterilization and observe the sterilization conditions specified. Conditions other than those specified may damage the product.

The drills are equipped with an inactive tip (only lateral cutting), have high durability in sharpening, and have a shape equivalent to the post. Thus, the double conical post fits to a double conical preparation, producing the stability needed for the system and attaching to the root with less cement thickness, which favors its long-term longevity.

When are intracanal posts indicated?

• When a dental element has been treated endodontically and the opening of access has significantly weakened it.
• When there is extensive coronal destruction and there is the need for a post to retain and/or restore the coronal structure.

Precautions and contraindications

• Posts may present exposed tips of the fiberglass which compose them, which can puncture the skin. Cutting the posts to adjust the length can also expose sharp fibers. Use safety glasses when cutting the posts and avoide cutting it near the patient’s face to prevent any accident or discomfort with waste from cutting. Avoid breathing the dust from cutting the posts.
• To handle the product, always wear latex gloves. Posts must be free of oily residues, organic materials, or any other contaminating agents at the time of cementation to prevent failure in adhesion.
• The use of dentinal adhesives over Whitepost is unnecessary and not recommended for FGM fiberglass posts, because the adhesive is a polymer with mechanical properties that are inferior to those of the post and resin cement, which produces an interface of low adhesive strength between the components.
• During the process of cementing the post previously silanized with Prosil, direct contact with the cement promotes a gradient of favorable mechanical properties, because •Whitepost has high mechanical properties and Allcem and Allcem Core, due to the loads, are stronger than a dentinal adhesive. •Therefore, the application of silane is sufficient to ensure anchoring between cement and post.

Preservation and Storage

• Always keep the product between 5º and 35ºC, in its original packaging, properly closed.
• The validity period of Whitepost is 5 years from the date of manufacture.
• Do not use the product after its expiration date.
